CANN MED, PC was founded in 2016 by Dr. Danny Hernández, who has been providing medical care to the community since 2002. He received his medical degree from Robert Wood Johnson Medical School, and completed his residency in Family Medicine at Medical College of Pennsylvania. He was awarded Atlantic County Medical Society Scholar Recipient and National Health Corp Loan Repayment Scholar honors. Dr. Hernandez is Board Certified in Family Medicine and as an Independent Medical Examiner. He is a member of the American Academy of Family Medicine and Pennsylvania Academy of Family Medicine.

Medical conditions that qualify for medical marijuana:
- Amyotrophic Lateral Sclerosis
- Autism
- Cancer
- Inflammatory Bowel Disease
- Crohn's Disease
- Damage to the nervous tissue of the spinal cord with objective neurological indication of intractable spasticity
- Epilepsy
- Glaucoma
- HIV (Human Immunodeficiency Virus) / AIDS (Acquired Immune Deficiency Syndrome)
- Huntington’s Disease
- Ulcerative Colitis
- Intractable Seizures
- Multiple Sclerosis
- Neuropathies
- Parkinson’s Disease
- Post-traumatic Stress Disorder
- Damage to the nervous tissue of the spinal cord with objective neurological indication of intractable spasticity
- Severe chronic or intractable pain of neuropathic origin or severe chronic or intractable pain in which conventional therapeutic intervention and opiate therapy is contraindicated or ineffective
- Sickle Cell Anemia

For Patients
Patients that want to participate in the medical marijuana program must follow these steps:
- Visit the Department of Health’s Patients and Caregivers Registry, and create a patient profile.
- Obtain medical records from a provider treating one or more of the illnesses listed.
- Receive a physician’s certification confirming that you suffer from one of the 17 serious medical conditions.
- Return to the Patient and Caregivers Registry and pay for a medical marijuana ID card. $50
- Obtain medical marijuana from an approved dispensary in Pennsylvania.
For Caregivers
Caregivers that want to participate in the medical marijuana program must follow these steps:
- Visit the Department of Health’s Patients and Caregivers Registry and create a caregiver profile.
- Complete a criminal background check.
- Complete an ‘Authorization to Designate a Third-Party Caregiver’ form, if necessary.
- Ensure your patient has a patient certification.
- Return to the Patient and Caregivers Registry and pay for a medical marijuana ID card.
